Our team consists of hard-working individuals who are professional, compassionate and dedicated to serving clients and their families facing life-limiting illness.

Hospice Nurses

Hospice RN’s provide clinical care with initial and ongoing comprehensive assessments. RN’s also supervise HHA’s and coordinate closely with the hospice physician and IDG team.

Hospice Aides

HHA’s provide personal healthcare and related activities for the client in their place of residence. They work under the direction supervision of an RN and follows the IDG’s plan of care.

Social Services

Our diverse team of Social Workers, Chaplains, and more assess and treat the patient’s psychosocial, environmental, spiritual, financial and other needs and helps coordinate care with the IDG team.
